SUBCONTRACTORS
Due to recent changes in CVHFH policies we are asking all subcontractors that wish to bid on our projects follow the listed requirements. This is in part due to Covid-19, and our desire to take a more professional approach to our building projects

A request for a quote, or Scope of Work, will be sent to a subcontractor. This Scope of Work will be a detailed outline of what CVHFH is asking you to bid on. If you decide to bid the following guidelines must be followed. 

Please sign and submit the below documents and materials;
  1.  Subcontractor Agreement including initials on item #9
  2.  CVHFH Covid -19 Exposure Prevention and Response Plan
  3.  A signed copy of our Whistleblower Policy
  4.  Your detailed bid for the Scope of Work requested by CVHFH
  5.  A project schedule
  6.  W-9 form
  7.  Certificate of Insurance
  8.  Workers Compensation Waiver, if applicable
